Case Manager (m/f/d) – Integration Support for Children with (Imminent) Disabilities at the Aachen Location

Your Responsibilities

As the agency responsible for integration assistance under the Federal Participation Act (BTHG), the LVR is also responsible for providing integration assistance services to children with (impending) disabilities. These are approved by Department 41 of the Division of Children, Youth, and Family as special education services in daycare centers and family daycare, as well as early intervention services under SGB IX


As a case manager, you will advise parents of children with (impending) disabilities, up until school entry, on the options for early intervention services as well as support options in daycare. As part of the needs assessment using the BEI_NRW KiJu needs assessment tool, you will assess the individual living situation and participation barriers of the children eligible for services. You determine the services best suited to meet the participation needs of a child with (impending) disabilities. In doing so, you ensure the social participation of children in early childhood education, support families’ self-determination, and thereby offer new perspectives. Counseling is provided in a low-threshold manner through our on-site counseling services at our numerous decentralized locations throughout the Rhineland.

As a Case Manager, you will be responsible for the following tasks in particular:

Advising and supporting families regarding integration assistance benefits and other support services (pursuant to §106 SGB IX)
Assessing participation needs using BEI_NRW KiJu (pursuant to Section 118 of SGB IX) in accordance with the ICF-CY
Recommending necessary integration assistance services and conducting impact assessments
Independently documenting and maintaining electronic administrative files
Networking with key stakeholders in the social sector, such as youth and social services offices, daycare centers, early intervention centers, and autism therapy centers
